Job Description

Summary

Under the direction of the Director of Nursing and in cooperation and collaboration with the interdisciplinary team, the Clinical Support Nurse supports the nursing staff with the admissions of new patients, orientation of new employees, and other duties as assigned by the Director of Nursing in accordance with current federal, state and local standards, guidelines or regulations, or established policies and procedures.

Essential Job Responsibilities

  • Staff will provide patient care, treatment, and services within the scope of their license, certification or registration and as required by law and regulation.
  • In coordination with the staff nurses, the Clinical Support Nurse will complete the following elements of all patient admissions
    • Orders (medication, treatment, diet)
    • Standing Order sets
    • Care plan, including but not limited to:
      • Potential for injury associated with falls
      • At risk for pain
      • Self-care deficit
      • At risk for skin breakdown
      • Hot liquid risk
      • Baseline care plan
      • Applicable co-morbidity care plans may include: diabetes, anticoagulants, respiratory, infection, wounds, urinary catheter
    • Resident Summary, including but not limited to:
      • Transfer Status (ADL section)
      • Shower Days/Times
      • Specific toileting needs
      • Diet and any other information pertinent to immediate care provisions
    • Medication Reconciliation
    • Review After Visit Summary and other discharge paperwork from referring facility to verify and confirm upcoming appointments
  • Primary preceptor for new employees electronic medical record training.
  • May be required to work as a staff nurse (i.e. direct patient care, medication administration, etc.) as needed
  • Fulfill responsibilities in Nursing Leadership on-call rotation as assigned.
  • Under the direction of the Director of Nursing/Administrator, assist with and/or independently complete other duties as assigned.
  • May be required to work beyond normal working hours, on weekends and other positions temporarily when necessary and may be subject to call back during emergency conditions or State/CMS survey activity. May be required to work shifts other than those for which normally assigned.

Essential Job Qualifications

  • Must have a current license to practice as a nurse in Michigan. Registered Nurse (RN) preferred; Licensed Practical Nurse (LPN) also accepted.
  • Current CPR certification is mandatory.
  • A minimum of two years' experience in long term care setting.
  • Must have excellent assessment, planning, organization, communication and interpersonal skills with an understanding of person centered care.
  • Ability to work flexible shifts including periodic weekends, holidays and off shifts.
  • Excellent verbal and written communication and possess the ability to express self clearly and effectively in an interdisciplinary care setting with residents, families, and staff members.
  • Ability to effectively present information and respond to questions from patients, staff, families, vendors, and managers.
  • Ability to define and solve problems, collect data, establish facts and draw valid conclusions and produce action plans.
  • Ability to interpret an extensive variety of technical journals, government requirements and regulations.
